Melanoma primario de mucosa nasosinusal y bucal: Revisión narrativa

Resumen

Los melanomas primarios de mucosa nasosinusal y bucal (MPMNSB) son tumores agresivos e infrecuentes; muestran un comportamiento biológico diferente al de su contraparte cutánea. Comprenden el 4% de cánceres de cabeza y cuello, y menos del 0.5% de todos los melanomas. Frecuentemente, los MPMNSB son diagnosticados en etapas avanzadas por la dificultad del paciente en reconocer signos y síntomas tempranos. Estudios recientes vinculan su tumorogénesis a alteraciones en diversos genes supresores de tumores y oncogenes que afectan vías de señalización como NRAS, KIT, BRAF, PTEN y MITF, influyendo en la proliferación, supervivencia y apoptosis celular. La identificación de estas alteraciones permite desarrollar terapias dirigidas, como inhibidores de BRAF y KIT (vemurafenib y sunitinib), aplicados actualmente en el melanoma cutáneo metastásico; sin embargo, la respuesta general no ha sido satisfactoria en melanomas de mucosa de cabeza y cuello, cuya sobrevida a cinco años varía entre el 15% y el 30%. Esta revisión bibliográfica tiene como objetivo proporcionar un recorrido integral sobre los aspectos clínicos, patológicos, moleculares y terapéuticos de los MPMNSB. Para ello, se llevó a cabo una revisión narrativa de la literatura publicada a nivel mundial utilizando las bases de datos PubMed y Google Académico. Se incluyeron artículos relevantes publicados entre 1990 y 2024, seleccionados con base en su pertinencia para evaluar aspectos clínicos, patológicos, moleculares y terapéuticos del MPMNSB. Para la estrategia de búsqueda se utilizaron las siguientes palabras clave: melanoma mucoso, melanoma oral, melanoma nasosinusal, inmunohistoquímica y terapias dirigidas.
